APPLICATION FOR MEMBERSHIP

We apply for Membership in the National Independent Service Organization. Our proper membership classification is indicated. Our initial fee and first year annual dues payment is enclosed (Membership dues must be submitted in U.S. funds) along with a signed Application for Membership. MEMBERSHIP CLASSIFICATION REGULAR MEMBERSHIP—INDEPENDENT SERVICE ORGANIZATION Membership is available to any organization meeting the following criteria:

Must be an established commercial "for profit" organization that provides service agreements for copiers and lasers, manufactured by, but not limited to, Xerox or Kodak with speeds of seventy (70) copies per minute or more. An independent service organization is a sole proprietorship, partnership, joint venture, corporation, or other business entity which is not legally or beneficially owned, either in whole or in part, by Xerox, Kodak or any other manufacturer of copying or laser printer equipment.

Initial fee is $500.00 plus first year’s annual dues (see categories below). The annual dues may vary depending upon the fee amount decided by the Board of Directors for all member-organizations for that year, and are due February 1st of every year. The second year dues will be pro-rated according to the month the initial fee was received from applicant.

Regular ISO Membership Category - Annual Dues:

1-99 Employees - $1,050.00

100-249 Employees - $2,200.00

250-499 Employees - $4,200.00

500 + Employees - $7,700.00

$1,000.00 VENDOR MEMBERSHIP

Membership is also available to any parts supplier meeting the following criteria:

Parts suppliers may become a Vendor Member if their primary business is the providing of parts or other materials which facilitate the maintenance of Xerox or Kodak copying or Laser equipment producing seventy (70) copies per minute or more. The Vendor Member cannot in any way be affiliated with either Xerox or Kodak Corporations. Initial annual fee is $1,000.00. The annual dues may vary depending upon the fee amount decided by the Board of Directors for all member-organizations for that year, and are due February 1st of every year. The second year dues will be pro-rated according to the month the initial fee was received from applicant. After the first year, Vendor annual dues will be $500.00 (Five Hundred Dollars), as set by the Board of Directors. Vendor Members will have limited rights. During all meetings, Vendors will be provided scheduled times for meetings with Regular N.I.S.O. Members; however, attendance to general meeting will not usually be allowed. Additionally, Vendor Members are not allowed to serve on the Board of Directors. The Board of Directors will have final determination regarding participation of Vendor Members in all events.

COMMON CAUSE All members are expected to promote the independent service of Xerox or Kodak High-speed copying or laser equipment and any action perceived by the Board of Directors to be contrary to this goal will result in immediate termination of membership with no refund of fees.The Board of Directors reserves the right to seek whatever information necessary from applicants to make a final determination that the membership criteria are met. We (applicant) are not associated or affiliated with either Xerox or Kodak Corporations.
